What kind is it or what is it made out of? Styrofoam painted like a log
Is there a UVB light? Yes, tho one was inoperable for a month and had to replace
What have you been trying to feed it? Variety of floating sticks, meal worms, river shrimp

How big is the tank/pond/enclosure? 40 gal, new since August but problem started prior to that

Is there any other unusual activity/symptoms? The shell is looking flaky but is still solid underneath the flakes. No squishy or white spots. I will try uploading a picture in the comments as it is too big. No other problems, pleasant and happy guy unaffaid of the kids, often swims towards me/them when near. Super man poses in the basking light even if the class is loud. Loves to tip over his ceramic/plastic bonsai tree, took him to the vet months ago for a check up and he said he looked a little underweight so I tried to increase feeding. Will occasionally give lettuce, carrots, apple peels, live feeder fish, natural aquatic plants, etc as well as floating stick food and “gourmet” treat medley.
