A trip to the vet will depend on the cost of the office visit for any individual vet. It will also vary based on any medications that may be given, or extras such as x-rays. I suggest finding the vet that you want to go to, preferably a herp vet, and call them to see. Tell them the nature of the visit and they can give you some estimates on cost for medications too.
As for sand. Some people use it, but there is still a risk of impaction. Anything that goes into the tank that is small enough to fit in the turtles mouth is likely to go there. I've never had any experience with sand, but I understand that it can be really hard on the filters. A bare tank bottom and/or a few pieces of river rock are a lot easier to deal with, imo.
