It's hard to tell if anything is wrong with his eyes like you said because the picture is blurry.

You might want tto revise your setup. That little gravel needs to be taken out. He could eat it and it makes the tank a lot harder to clean because it traps waste and food. Also it looks like he's only got a couple inches of water. You should fill your tank up as much as you can (probably about 3/4 would be good) and get a floating dock for your turtle to bask on.
