do you keep the UVB light on all day/night? and how far/close do you put it? I heard if you put it too close, it could cause the turtle to have swollen eyes

about 12 hours a day and the heat lamp close enought to keep in a consent 90ish degrees and the UVB it depends on how strong it is.

turtles need sleep, too so turn it off at night
