Is your turtle deficient in vitamin A? If not, adding vegetables like carrots will be fine for covering the vit a requirement. I wouldn't supplement it with additional vit a unless there's an issue like swollen eyes indicating something's wrong.
I also would lower the amount of protein in the diet. Shrimp and trout pellets is too much protein for a turtle over 4inches. Once turtles stop their initial growth spurt they don't need as much protein and overfeeding it will cause organ stress and pyramiding. I'd stick with just the pellets and only give them every other day. Shrimp is fine as a once a month treat, but I wouldn't offer it any more frequently than that.
Finally, spinach is not recommended for turtles because of it's high oxalic acid content which can block calcium absorption, so I'd cut that out of the vegetable part of the diet.
