Sugar free means that it contains an artificial sweetener, along with whatever chemical mix makes the flavoring. I can't eat anything sugar free because I'm allergic to artificial sweeteners- there are a growing number of studies showing that they're not really healthy for you. So I wouldn't try it for my turtles as I have no idea what it would do. But for diabetics, yeah, limited options.
Turtles don't get a lot of sugar in their natural habitat, and especially not refined sugar. They'd probably have trouble digesting it, they have a slow metabolism and don't need a lot of quick energy the sugar would provide. Also, it would change the flavor of the jello shot from "yum tastes like fish and greens" to "sugary". Part of the purpose of the shots is to get them used to the flavor of more good for them greens by making it taste like fish and tricking them into acclimating.
